Consent of Use of Personal Information

I understand that in order for Legal Expenses Insurance Southern Africa Limited (LEZA) and its South African subsidiaries; Scorpion Legal Protection (RF) (Pty) Ltd, LegalWise South Africa (RF) (Pty) Ltd, , Family Insurance (Pty) Ltd, UGO Selling (Pty) Ltd and Yes Loans (RF) (Pty) Ltd to positively verify my identity for the purposes of obtaining a Government Payroll Deduction, applying for insurance, normal Membership alteration, or claims processing, my Personal Information must be accessed.

I also understand that LEZA may verify my identity either through a method of asking questions pertaining to my Personal Information or by using biometric verification solutions. My answers or biometric data will be shared with the Authorised Verification Agent, who will confirm whether the verification is correct or not. LEZA will not see or receive the correct answers to the verification questions, and will receive a final positive or negative verification from the Authorised Verification Agent.

I hereby authorise LEZA and its duly Authorised Verification Agent, to access my Personal Information and conduct background screening checks.

I consent to requests for consumer credit information to be released for fraud prevention or detection purposes.

I agree that any Personal Information supplied to LEZA is provided voluntarily and that LEZA may not be able to comply with its obligations if the correct Personal Information is not supplied.

I understand that privacy is important and that reasonable efforts are in place to ensure that any Personal Information in possession or processed is kept confidential, stored in a secure manner and processed in terms legislation and for the purposes I have authorised.

I warrant that all information supplied is accurate and current and agree to correct and update such information when necessary.

By submitting any Personal Information to the LEZA Group, its subsidiaries and/or Responsible Party in any in any form, I acknowledge that such conduct constitutes a reasonable unconditional, specific, informed and voluntary consent to the processing of such Personal Information in the manner set out below by the Responsible Party and/or Verification Information Suppliers, subject to my rights in terms of applicable data protection legislation, including the right to withdraw such consent where permitted by law:

• Personal Information may be shared with the Verification Information Suppliers for verification or other legitimate purposes and be stored for a reasonable period by the Responsible Party and/or the Verification Information Suppliers.

If the Responsible Party utilises the Personal Information contrary to the Privacy and Data Protection Conditions, I may first resolve any concerns with that Responsible Party. If I am not satisfied with such process, I have the right to lodge a complaint with the Information Regulator. I agree to indemnify the Responsible Party, and Verification Information Suppliers, acting in good faith in taking reasonable steps to process my Personal Information lawfully, against any liability that may result from the processing of my Personal Information. This includes unintentional disclosures of such Personal Information to, or access by unauthorised persons, and/or any reliance which may inadvertently be placed on inaccurate, misleading, or outdated Personal Information, provided to LEZA by myself or by a third party in respect of me.

I understand that it is a condition of Responsible Party's Verification Information Suppliers that this information is furnished by them solely for the purposes of verifying my credentials.

I agree that this consent and indemnity declaration remains valid for all current and future personal credential verification requests by WHOYOU (PTY) LTD, as an Authorised Verification Agent.

I acknowledge that I may withdraw such consent at any time by notifying the Responsible Party in writing. I understand that such withdrawal will not affect the lawfulness of processing undertaken prior to the withdrawal and that the Responsible Party may continue to process or retain Personal Information where required by law, regulation or legitimate operational requirements.

By providing my personal and biometric information I consent to this information being stored for subsequent verification and protected as set out in the Privacy and Security Policy.
