Consent of Use of Personal Information

I understand that in order for the Company/ Scorpion Legal Protection to positively verify my identity for the purposes of obtaining authorisation for a Government Payroll Deduction and application for insurance, my personal information must be accessed.

I also understand that Scorpion Legal Protection will verify my identity through a method of asking me questions pertaining to my personal information and that my answers will be shared with the Authorised Verification Agent who will confirm whether the answers are correct or not. Scorpion Legal Protection will not see or receive the correct answer, and will only receive a final positive or negative verification from the Authorised Verification Agent.

I hereby authorise Scorpion Legal Protection and its duly Authorised Verification Agent to access my personal information and conduct background screening checks.

I consent to requests for consumer credit information to be released for fraud prevention or detection purposes.

I acknowledge that any personal information supplied to Scorpion Legal Protection is provided voluntarily and that Scorpion Legal Protection may not be able to comply with its obligations if the correct personal information is not supplied.

I understand that privacy is important and that reasonable efforts are in place to ensure that any personal information in possession or processed is kept confidential, stored in a secure manner and processed in terms of legislation and for the purposes I have authorised.

I warrant that all information supplied is accurate and current and agree to correct and update such information when necessary.

By submitting any personal information to the Company and/or Responsible Party in any form, I acknowledge that such conduct constitutes a reasonable unconditional, specific and voluntary consent to the processing of such personal information in the following manner by the Responsible Party and/or verification information suppliers:

• Personal information may be shared with the Verification Information Suppliers for verification or other legitimate purposes and be stored for a reasonable period by the Responsible Party and/or the Verification Information Suppliers.

If the Responsible Party utilises the personal information contrary to the Privacy Policy, I may first resolve any concerns with that Responsible Party. If I am not satisfied with such process, I have the right to lodge a complaint with the Information Regulator. I agree to indemnify the Responsible Party, and Verification Information Suppliers, acting in good faith in taking reasonable steps to process my personal information lawfully, against any liability that may result from the processing of my personal information. This includes unintentional disclosures of such personal information to, or access by unauthorised persons, and/or any reliance which may inadvertently be placed on inaccurate, misleading, or outdated personal information, provided to the Company by myself or by a third party in respect of me.

I understand that it is a condition of the Responsible Party's verification information suppliers that this information is furnished by them solely for the purposes of verifying my credentials.

I agree that this consent and indemnity declaration remains valid for all current and future personal credential verification requests by Fides.

By providing my personal and biometric information, I consent to this information being stored for subsequent verification and protected as set out in the Privacy Policy.
